About the Program
This is a supportive wound care program created to help people with chronic or hard-to-heal wounds. We work with licensed providers across South Florida to deliver advanced wound treatments in a safe and convenient way.

How Care is Provided

• In the comfort of your home

• At your assisted living facility ALF or rehab centers

• Or with free transportation to one of our partner clinics
  ​

Most participants receive weekly visits for wound care, follow-up, and progress checks.

Who May Qualify
 

You may be eligible if:

• You have a wound that has not healed after 4 or more weeks, such as:

  • Diabetic foot ulcer

  • Venous leg ulcer

  • Pressure ulcer (bed sore)

  • Post-surgical wound
     

• You are able to receive care at home or attend scheduled visits

• You have Medicare Part B
